GP and Health Professional Referrals

We work closely with GPs and health professionals, recognising the vital role you play in supporting patients and families through both aged care and palliative care needs. At Florence at Home, our goal is to make the referral process simple, secure, and collaborative.

By referring to us, you can be confident that your patient will receive:

  • RN-led continuity of care with one nurse who knows them and their family’s needs

  • Holistic support that includes coordination with GPs, specialists, allied health, and community services

  • Clear communication back to you as the primary care provider, ensuring you are updated and involved in all major decisions

  • Timely care that can begin privately while My Aged Care referrals or assessments are pending

Referrals can be submitted securely through our online referral form, or you are welcome to contact us directly to discuss your patient’s needs.
