Aged Care
at Home

For older Australians (65+) who want to keep living safely, comfortably, and with dignity in their own homes.

With RN-led care, we provide:

  • Early detection & care planning - RN assessments that spot changes quickly, guide safe medication use, and recommend timely reviews
  • Referrals & coordination - we know when to bring in physio, OT, dietetics, or other specialists, and make sure everyone’s on the same page.
  • Home safety & enablement - practical RN review, coordination of equipment hire/purchase, and referral pathways to OT for complex needs.
  • Evidence-based support - clear, current, and person-centred care that adapts as your needs change.

We work closely with your GP and allied health providers as part of a multidisciplinary team, and our services align with the five in-home support categories under government funding.

Funding: Available through My Aged Care (65+). We can guide you through assessments and the upcoming Support at Home program.

With RN-led care, we provide:

  • Symptom and medication management

  • Education, guidance, and reassurance for families and carers

  • After-hours support when needed

We coordinate with GPs, hospitals, and allied health teams so care is consistent and centred on your loved one’s wishes.

Access: Available for all ages. Private pay is available, and we’ll talk you through the best options for your family.
