We use JotForm, a secure and professional platform, to make referrals quick and hassle-free for GPs and practice staff. The form is designed so you can copy and paste directly from your clinical software, reducing double handling and saving valuable time.

All information is transmitted securely and handled in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles, giving you confidence that referrals are both efficient and safe. Referral details are collected solely for the purpose of coordinating care and will never be shared with third parties without the patient’s consent.
