Care with Heart

Florence at Home puts choice back into the heart of aged and palliative care — because every journey deserves to finish where it began: at home. Guided by the principles of Florence Nightingale, we bring a compassionate, holistic approach back to nursing: care that values dignity, safety, and connection.

Founded by Fiona Antony, a highly experienced nurse with over 13 years in palliative and community nursing care, our service exists to fill the gap in after-hours and in-home specialist support across Portland and the surrounding region.

Every visit is built on trust, respect, and the belief that health care should meet people where they are — both physically and emotionally.

Helping People Stay at Home for Longer

Most families want the same thing for their loved ones: to remain safe, comfortable, and cared for at home, for as long as possible.

With changes coming to the aged care system, many everyday supports like meals, showers, mobility assistance, and social connection are being labelled as “non-essential.” For families, that often means more out-of-pocket costs.

At Florence at Home, we believe these supports are not optional. They are essential care that keeps people well, prevents hospital visits, and helps them hold on to independence for longer.

Because our care is RN-led, every visit recognises the value of these supports as genuine healthcare. That means families can feel confident their loved one is receiving the right care at the right time — care that protects dignity, reduces risks, and allows them to stay at home, where they belong.

Helping People Stay at Home for Longer

  • The aged care system is changing, and families are being asked to pay more out of pocket for things that matter most — meals, showers, mobility help, even companionship. On paper these are called "non-essential," but for families they are the very supports that keep loved ones safe and at home.

At Florence at Home, we see them for what they really are: essential care that protects dignity, independence, and wellbeing. Because our care is RN-led, these supports are recognised as clinical care, not extras. That means less financial strain for families and more time at home for the people you love.

For this reason, we offer palliative and end-of-life support for those under 65. We directly provide:

Behind Every Number is a Family’s Story

Behind every statistic is a family doing their best to keep a loved one safe, comfortable and connected. Too often, older Australians face long delays, preventable risks, and the loss of independence when forced to leave their homes.

At Florence at Home, we provide a different path. With a registered nurse leading the care, we focus on practical solutions that reduce risks like falls, ease the pressure of waiting for services, and honour each person’s wish to remain at home.

Our mission is simple: to help families feel supported and to give older Australians the dignity, safety and choice they deserve — right where they belong.

How it Works

Every family’s journey looks a little different, but the steps to receiving care don’t need to feel overwhelming. At Florence at Home, we guide you through the process so support arrives with ease and compassion.

Applying early matters — the My Aged Care process can take several weeks (1–2 weeks for the first call, 2–8 weeks for an assessment, and another 2–6 weeks for approval), and can be longer depending on demand and waiting lists. Starting sooner gives you the best chance of having care in place when you need it most.

For Australians 65 and over
Government-funded in-home aged care starts with a My Aged Care referral. Once you’ve been referred, we make the journey simple:

Access:
A registered nurse takes the time to understand your health needs, daily routines and personal wishes.

Plan:
Together, we design a care plan that aligns with the government’s Support at Home categories.

Care:
Our nurses bring that plan to life, providing safe and compassionate support in the comfort of your home.

Support:
We coordinate additional services such as meals, transport and allied health, so your care feels seamless and complete.

For those under 65
For individuals under 65, or when timely access to My Aged Care is not possible, Florence at Home offers private palliative and end-of-life care. Services can be accessed directly, with support commencing immediately.

Advance Care Planning

Planning ahead is one of the greatest gifts you can give yourself and your family. Advance Care Planning is not just paperwork — it’s a way of making sure your wishes are heard and respected, even if there comes a time when you can’t speak for yourself.

It’s about more than medical decisions — it’s about recording your values, preferences and goals for care, and giving your loved ones clear guidance so they don’t have to make difficult choices alone. Having these conversations early, and putting your wishes in writing, provides peace of mind for you and reassurance for those who care about you most.

Journeys We’ve
Walked Together

Every family’s needs are unique, and no two journeys look the same. That’s why we share both case studies and testimonials — so you can see how care works in practice and also hear directly from the people we’ve walked alongside.

Our case studies outline real-life situations and how support was tailored to meet each person’s needs, while testimonials give voice to clients and families in their own words. Together, they offer an honest picture of the difference compassionate, RN-led care can make at home.
